What the consultation form collects
We collect the contact and business details you provide, including your name, business name, email, phone number, website, location, products or services, revenue range, previous grant response, requested service and optional comments. We record the date of your inquiry.
We also record source and campaign tags, the landing page and the referring page when available, to understand which promotions bring inquiries. Referring page query strings are not included in the lead record.
Why we collect it
We use this information to review your consultation inquiry, discuss eligibility, respond to you, arrange a consultation and manage follow-up. An inquiry does not guarantee eligibility or grant approval.
Storage and access
Inquiries are stored in our WordPress database and sent to our consultation team through our hosting and email systems. Stored inquiries are accessible to authorized site administrators. An acknowledgment is sent to the email address you provide.
Inquiries remain in the database until an administrator removes them. Website backups may contain copies of stored inquiries. Please do not submit passwords, payment details, tax documents or other sensitive documents through this form.
Spam protection and cookies
The form uses spam checks and a temporary identifier derived from an IP address to limit repeated submissions. This identifier expires after one hour without another accepted submission; the IP address is not saved as a field in your inquiry. WordPress may use authentication cookies when site administrators sign in.
Booking and external websites
The consultation calendar is provided by Calendly and can appear directly on our thank-you page. When you view the embedded calendar, your browser connects to Calendly. Information you enter there is handled by Calendly as well as the meeting organizer. You can review Calendly's privacy notice. Recipient website links open external websites with their own privacy practices.
Questions or removal requests
Call (866) 306-9861 to ask about information you submitted, request a correction or request its removal. Please include enough information to help us locate your inquiry without sending sensitive documents.
